Joining The Practice

People who want to join this practice should come to the Reception Desk during surgery times.  You should bring your NHS medical card (if available), together with proof of residence (a document showing your name & address).

When new people join the practice we ask for a health check form to be completed.

If you want to leave the practice you do not have to ask the doctor’s permission, simply go along and register at the new surgery of your choice.

Practice Area

Our practice boundary is St Mary’s Road/Granville Road running down City Road/Ridgeway Road, onto Norton Avenue, across onto Hemsworth Road/Cobnar Road and up Chesterfield Road, crossing Abbeydale Road, back up to St Mary’s Road by Union Road & Cemetry Road. If you are in any doubt please check with our reception staff.

Which Doctor Should I see?

You can choose to consult any of the doctors, as all Doctors have full access to your medical records. If you have an urgent medical condition you can see any doctor or other Health Professional for immediate treatment.

To save time you may want to print off a registration form, fill it out and bring it in with you together with your NHS Medical Card (if available), and proof of your address (up to date bill or any recent document showing your name and address).
